29 CFR 1910.252(a)(2)(ii): Fire extinguishers. Suitable fire extinguishing equipment shall be maintained in a state of readiness for instant use. Such equipment may consist of pails of water, buckets of sand, hose or portable extinguishers depending upon the nature and quantity of the combustible material exposed. a. Fabrication Department/Welding Cells - On or about October 1, 2010, employees were required to perform welding operations and fire extinguishing equipment was not maintained in a state or readiness for instant use. The welding Cells location that were missing suitable fire extinguishing equipment were the following; 1. The welding cell unit on the east wall did not have fire extinguisher for instant use. 2. The welding cell unit on the southeast wall did not have fire extinguisher for instant use.

29 CFR 1910.252(c)(1)(ii): Screens. When welding must be performed in a space entirely screened on all sides, the screens shall be so arranged that no serious restriction of ventilation exists. It is desirable to have the screens so mounted that they are about 2 feet (0.61 m) above the floor unless the work is performed at so low a level that the screen must be extended nearer to the floor to protect nearby workers from the glare of welding. a. Fabrication Department - On or about October 1, 2012, employees were required to walk through an area where welders were not entirely screened on all sides and the conditions of the screens were the following; 1. Some portable welding screens had rips, holes, and tears on their clothing material. 2. Several portable welding screens were missing their tie straps on all tubular sides. 3. Two portable welding screens were missing part of their floor base's tubular frame.

Fabrication Department - On or about October 3, 2012, employees were required to conduct welding operations and to use fire extinguishers were not provided with an educational program at least annually. Employees were potentially exposed to the hazards of smoke inhalation and fire.
