29 CFR 1910.176(b): Storage of material created a hazard: a. North Turning Rolls West Rack - On or about January 9, 2024, Naylor Pipe Company failed to ensure employee(s) were protected from being struck and crushed by a pipe when preparing to hook the pipe to be lifted and transported by another employee operating the Whiting 20-ton overhead gantry crane. The employee stood on the side of the stacked pipe, in an aisleway, to remove the chock, manually position the pipe and hook the 16-foot metal cable sling around the north end of the 14-inch OD 55-foot-long pipe weighing approximately 2500 pounds. When the chock on the north end of the pipe was removed, the affected employee was exposed to struck-by and crushing hazards when the pipe rolled and fell three (3) feet off the rack, as the existing posts were too short to stop the rolling pipe and prevent it from falling.
