29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Side of residential structure: On September 17, 2012 an employee working on the ground with/below another employee who was performing sheathing activities on the roof was not wearing a hard hat.      b) Backside of residential structure: On September 17, 2012 an employee working on the ground with/below another employee who was installing "jacks" (trusses) was not wearing a hard hat.      Note) Abatement certification is NOT required for this item.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      a) Front side of structure:  On September 17, 2012 and at times prior thereto an employee performed sheathing activities on a 8 in 12 pitched roof without a fall protection system in place.       b) Back side of structure: On September 17, 2012 an employee performed truss "jack" setting activites while standing on the top of a window wall without a fall protection system in place.      Note) Abatement certification IS required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:      a) Jobsite: On September 17, 2012 and at times prior thereto the responsible person on site and employees performing framing activities did not recognize and/or acknowledge the hazards of performing framing activities without a fall protection system in place, and take the appropriate actions to minimize these hazards.  Framing activities included sheathing and setting trusses.      Note) Abatement certification IS required for this item.
