29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:      a) On or about 10/1/12, on construction site at 180 Redtail Rd., Orchard Park, NY: A Skilsaw 3310 Table Saw was used to cut "tongue & groove" material for the ceiling. The Saw was connected to 120- volt ac power via a series of three extension cords, the first of which was plugged into a receptacle on the outside of the building. The plugs on the first and third extension cords in the series lacked a grounding prong.    b) On or about 10/1/12, on construction site at 180 Redtail Rd., Orchard Park, NY: A Makita Mitre Saw, Model LS1214F was used to cut "tongue & groove" material for the ceiling. The Saw was connected to 120-volt ac power via a series of two extension cords, the first of which was plugged into a receptacle on the outside of the building. The plug on the first extension cord in the series lacked a grounding prong.    NO ABATEMENT CERTIFICATION REQUIRED
