29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about at 10/03/12 at 3356 Lawrenceville Hwy, Tucker, GA: Employees were climbing the scaffold frame to access the first and second buck of the scaffold, no ladder was provided, exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: On or about at 10/03/12 at 3356 Lawrenceville Hwy, Tucker, GA: Employees were using the scaffold, without any guardrails on the open ends. The employees were laying brick 13.3' from the lower level and could fall off without the end or side guard rails.
