29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) Gonzalez Construction at 2431 Perry St. Denver, CO 80212: On or before 8/2/2017 the employer did not ensure that employees were protected from projectiles to the eyes. Employees were cutting plywood using circular saws, and nailing the plywood down while using pneumatic nail guns, without safety glasses. This condition exposed employees to the hazard of projectiles to the eyes from nails, flying chips, and splinters.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) Gonzalez Construction at 2431 Perry St. Denver, CO 80212: On or before 8/2/2017 the employer did not ensure that employees were protected from fall hazards while working on a 7/12 pitched residential roof. Employees were redecking this residential roof without the use of personal fall arrest systems. This condition exposed employees to fall hazards of nine feet.
