29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): Gonzalez Construction, Inc. - on or about October 3, 2012, an employee was required to use a filtering face-piece respirator when mixing cement and mortar. The employee had not received a fit test to ensure proper wearing of the respirator and adequate protection from airborne inhalation hazards.
