29 CFR 1910.178(l)(4)(iii):     An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.    The employer is failing to protect employees from potential struck by hazards associated with employees operating powered industrial trucks without having been certified within the past three years.  This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has uncertified employees operating propane powered forklifts used in moving product and materials across the facility without having been evaluated by a company approved trainer to certify their abilities.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    The employer is failing to protect employees from potential point of operation and amputation hazards associated with employees using a lathe with a missing guard.  This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has exposed employees using a Sumit horizontal lathe to make steel pins without having a guard to protect the operator for the fast spinning point of operation.

29 CFR 1910.215(a)(4):     Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    The employer is failing to protect employees from potential struck by hazards associated with employees using a bench grinder with an improperly adjusted work rest.  This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has exposed employees using the DeWalt model DW758 bench grinder to sharpen tools used in the CNC machines and the work rest is an inch from the grinding wheel creating the potential for a tool to be pulled into the grinder causing the wheel to explode and injure the employee.

The employer is failing to protect employees from potential electrical hazards associated with the improper use of electrical components.  This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has exposed employees operating the hydraulic/electric pump used to rotate the horizontal chassis jig in the chassis area and the employer has installed a light switch designed to be mounted to the wall as a substitute on/off switch.

This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has exposed employees operating the hydraulic/electric pump used to rotate the horizontal chassis jig in the chassis area and the employer has the pump plugged into an outlet using a temporary flexible cord that has been in constant use for more than 7 years.

The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.    The employer is failing to protect employees from potential caught between and electrical hazards associated with the lack of certification of the annual periodic reviews for the written lockout and tag out procedures.  This was most recently evident during the inspection of the facility located at 1708 H Street, Thurston, NE 68062, where the employer has employees locking out equipment using written specific procedures whereby the employer has not certified the annual periodic review was conducted and that the procedure in place is accurate.
