In this case, a shop fabricated sling device had elongated rings which also showed signs of wear. The sling device was used for a lift where other components failed due to insufficient capacity for the weight of the lift. Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility: The employer has failed to ensure that slings and fastenings were marked with some type of durable identification such as a tag to show the items listed above. On August 6, 2015, employees used a sling and other components to lift a load without knowing the capacity of the items used to facilitate the lift. None of the components were labeled as required. 29 CFR 1910.184(e)(2)(ii): Makeshift link(s) or fastener(s) formed from bolts, or other such attachment(s) were used to connect links or fastenings on alloy steel chain sling(s): Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility: The employer has failed to that slings used for lifting did not have components which were makeshift or not designed for that purpose. In this case, a sling used in a lift on August 6, 2015, was fabricated using components which were not designed for lifting. The main ring of the sling appears to have been made using a section of alloy piping cross cut to create a ring. Additionally, the alloy chain was welded to the ring which may have had a negative effect to the lifting capacity. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
