29 CFR 1910.147(c)(4)(ii):     The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:      Work site located at 1708 H Ave, Thurston, Nebraska and Throughout the facility:  Lockout/tagout procedures did not meet the requirements outlined above.  The developed procedures are vague in that they do not clearly specify the techniques in which lockout operations will be performed and they do not clearly indicate the locations where energy may be isolated (switches, valves, breaker numbers, etc).          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.184(c)(9):     Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s):    Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility:  The employer has failed to ensure that employees were kept clear of suspended loads.  On August 6, 2015, two employees sustained injuries when a suspended load fell due to failed components used in the rigging.  The employees were installing covers over a trailer dollys components prior to the dolly going into the paint area.         Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.184(d):     Sling(s) and fastenings were not inspected each day or during use and were not removed from service when damaged or defective:    Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility:  The employer has failed to ensure that slings and fastenings were inspected before use and removed when deficiencies were noted.  In this case, a shop fabricated sling device had elongated rings which also showed signs of wear.  The sling device was used for a lift where other components failed due to insufficient capacity for the weight of the lift.         Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility:  The employer has failed to ensure that slings and fastenings were marked with some type of durable identification such as a tag to show the items listed above.  On August 6, 2015, employees used a sling and other components to lift a load without knowing the capacity of the items used to facilitate the lift.  None of the components were labeled as required.         Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.184(e)(2)(ii):     Makeshift link(s) or fastener(s) formed from bolts, or other such attachment(s) were used to connect links or fastenings on alloy steel chain sling(s):    Work site located at 1708 H Ave, Thurston, Nebraska and throughout the facility:  The employer has failed to that slings used for lifting did not have components which were makeshift or not designed for that purpose.  In this case, a sling used in a lift on August 6, 2015, was fabricated using components which were not designed for lifting.  The main ring of the sling appears to have been made using a section of alloy piping cross cut to create a ring.  Additionally, the alloy chain was welded to the ring which may have had a negative effect to the lifting capacity.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The sling in question had been in use at the site for 20 years.        Abatement certification and abatement documentation is required for this violation.
