29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) North wall of the office trailer: A relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, T.Vs stereos, printers, etc., was instead used to provide power to high powered loads, such as a Black and Decker coffee pot; Frigidaire mini refrigerator and a Rival toaster. b) East wall of the office trailer: A relocatable power tap designed and listed to provide power to high concentrations of low powered loads such as computers, T.Vs stereos, printers, etc., was instead used to provide power to high powered loads, such as an air conditioner.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, and fittings were not protected from abrasion. a) West end of the 40 foot intermodal container used for storage: A job-made receptacle outlet on the outside of the container was equipped with a male plug end. The three individual conductors which entered into conduit and a metal-cased junction box on the inner wall were not fully enclosed and they were not protected from abrasion. ABATEMENT CERTIFICATION IS REQUIRED.

ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1926.405(g)(1)(iii): Unless necessary for a use permitted in paragraph (g)(1)(i) of this section, flexible cords and cables shall not be used where run through holes in walls, ceilings, or floors and/or used in lieu of fixed wiring. a) Northwest end of Office Trailer: A 100 foot; 120 volt extension cord ran from a receptacle outlet in the office trailer through a one-inch opening cut into the floor. b) West end of the worksite: A 120 volt extension cord was used in lieu of permanent wiring when it was run as a branch circuit to provide electrical power from the office trailer to the 40 foot intermodal container used to store plumbing supplies.
