29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On or before February 22, 2017, where a fan was being used in the basement parking garage, the strain relief, at the male end of the cord, was pulled away from the outer insulating jacket of the cord allowing the pull to be directly transmitted to joints or terminal screws, exposing the employees to an 112 volt electrical contact hazard.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a. On or before February 22, 2017, where a fan was being used in the basement parking garage, the ground prong was missing from the male plug of the cord, thereby exposing employees to an electrical contact hazard. Sauer Group Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1030107, citation number 1, item number 1b and was affirmed as a final order on April 8, 2015, with respect to a workplace located at 160 West Woodruff, Columbus, OH, 43210.
