On or before February 22, 2017, where a fan was being used in the basement parking garage, the strain relief, at the male end of the cord, was pulled away from the outer insulating jacket of the cord allowing the pull to be directly transmitted to joints or terminal screws, exposing the employees to an 112 volt electrical contact hazard.
