On the mechanical screen roof where the plumbers installed pipes for equipment, covers were not placed over the 4-6 inch holes thereby exposing employees to a trip fall hazard.

On the roof of the mechanical screening where four six inch covers were missing from the pipes on the roof between the roof and the floor below, protection was not provided for the employees working on the floor below thereby exposing employees to a struck-by hazards.
