Maya Brothers,Inc., 219 Second Avenue, Warren, Pa- On or about October 11, 2012, and at times prior thereto, company employees worked with and around hazardous chemicals including but not limited to, Portland Cement. The company did not provide information and training on the physical and health hazards fo the chemicals in the work area.
