OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    A.) Maya Brothers, Inc., 2020 Edinboro Road, Erie, PA  16509, first floor, on or about October 2, 2014 and times prior thereto, employees were exposed to impalement hazards from bolts that protruded from the concrete floor.  Employees on-site were performing work duties including, but not limited to, cleaning out holes and installing leveling plates on the first floor of the structure.  One-half inch bolts were cemented into the concrete floor, and protruded 2-3 inches out of the concrete floor to secure wood stud wall structures to the first floor.  The protruding bolts were not protected by impalement protection, including, but limited to, caps, cover and troughs.
