29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    1. Throughout the plant, at the time of the inspection, the employer was operating several multiple energy source (electric and pneumatic) machines without a machine specific energy control (lockout/tagout) program in place.

29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    1. At the time of the inspection, there were six (6) employees operating fork trucks who had not been trained and/or certified to drive fork trucks.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    1. Inside of the machine shop, there was a Nels Well Machine Tools drill press being used with no chuck (arbor) guard in place. Employees were exposed to caught in and struck by hazards.

29 CFR 1910.212(b):     Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving.    1. Inside of the maintenance shop, employees were using a Nels Well Machine Tools drill press that was not anchored to the metal table to prevent movement. Employees were exposed to caught in and struck by hazards.

29 CFR 1904.1(a)(2):     If your company had more than ten (10) employees at any time during the last calendar year, you must keep OSHA injury and illness records unless your establishment is classified as a partially exempt industry under �� 1904.2.    a) At the time of the inspection, the employer did not have the OSHA 300 logs available for review for the years 2012, 2011, and 2010. OSHA 300 logs were not available for the years 2008 and 2009 either.
