29 CFR 1910.147(c)(4)(ii):   The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  Zone Enterprises did not have specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy for machines with multiple energy sources.  Two Bridgeport milling machines, two LeBlond horizontal lathes, and  two Atom CNC Machines, model #s S530CN located in the plant did not have specific procedural steps for locking/tagging out.

On 2/4/2014, employees from Zone Enterprises were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On 2/4/2014, an employee from Zone Enterprises was exposed to point of operation hazards.  A table saw, serial # 95H9721, was in the maintenance area and did not have a guard in place, which completely enclosed the portion of the blade above the table or the material being cut.

Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.  On 2/4/2014, an employee from Zone Enterprises was exposed to struck-by hazards.   The tongue guards on a pedestal grinder in the sanding/grinding area were not adjusted to one-fourth inch.
