29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided a) On or about 10/18/12 at the new Science Center construction project at SUNY Fredonia, Fredonia, New York; employees were on a flat roof of the 3 story building beginning roofing work and activities. Employees were accessing the roof with an extension ladder and stepping on to the roof from the parapet. The parapet was 22 inches high. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(10): ALEX ROOFING COMPANY INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311877039, CITATION NO. 1, ITEM NO. 2A, AND WAS AFFIRMED AS A FINAL ORDER ON 7/1/10, WITH RESPECT TO A WORKPLACE LOCATED AT 1644 EAST 12TH STREET, ERIE, PA. Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system; a) On or about 10/18/12 at the new Science Center construction project at SUNY Fredonia, Fredonia, New York; employees were on a flat roof of the 3 story building beginning roofing work and activities. Fall protection systems and equipment were not being utilized. NO ABATEMENT CERTIFICATION REQUIRED
