29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On October 17, 2012 at a jobsite located at 34 W. Grove Street, Lombard IL, an employee was performing work from a residential roof and was exposed to a fall hazard of approximately 11.9 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    Fireplace and Chimney Authority Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 314343401, citation number 01, item number 01, and was affirmed as a final order on 04/26/2010, with respect to a workplace located at 1419 Elmwood, Evanston, IL.

29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:     On October 17, 2012 at a jobsite located at 34 W. Grove Street, Lombard IL, employees were working with Spec Mix mortar and did not know the hazardous associated with the mortar. Employees were not adequately trained on hazards associated with chemicals.      Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
