Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. On May 21, 2013, an employee was exposed to the hazards associated with a scaffold collapse as the employee worked on a tubular frame scaffold 24 feet above grade that was not restrained from tipping. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

FIREPLACE AND CHIMNEY AUTHORITY, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(1)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 108946, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON December 27, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 526 S. Harvey, Oak Park, IL 60302. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
