29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) At the project site on or about October 19, 2012, and times prior thereto, there was no strain relief for the female end of a 50 extension cord. The strained insulation of the cord was pulled away from the female plug. The cord was used to supply power to a DeWALT DW511drill.

29 CFR 1926.453(b)(2)(iii): Employee(s) were not prohibited from belting off to an adjacent pole, structure, or equipment while working from an aerial lift: (a) At the front of the Pacifica Plaza building on or about October 19, 2012, and at times prior thereto, an employee operating a Snorkelift, Model UNO-41E, Serial No. 9523670795, was tied off to a rope grab on the roof top of the building.
