29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  At the site - an employee involved in carpentry work was observed standing on a 2X4 that measured 5 foot long without a means of fall protection, exposing him to an 8 foot fall, on or about 10/18/2012.    THE FIRST CONSTRUCTION SERVICES, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316073626, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON 10/14/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 3200 N. ALAFAYA TRAIL, ORLANDO, FL.       THE FIRST CONSTRUCTION SERVICES, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(1),WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316306877, CITATION NUMBER 01, ITEM NUMBER 001 AND WAS AFFIRMED AS FINAL ORDER  ON 11/03/011, WITH RESPECT TO A WORKPLACE LOCATED AT HOLMSWOOD GREENS, LITHIA FL.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) At the site - in the interior of the building employees involved in carpentry work were observed climbing a 20 foot metal extension ladder that was not extended above the work platform at least 3 feet, exposing employees to an 8 foot fall, 10/18/12.
