29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) At the construction site, an employee was standing in a wooden plank of 12 inches wide at 91.5" (7'-6") high with a personal fall arrest system tied off with a 4 foot lanyard with shock absorber, on or about December 11, 2013.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: (a) At the north wing of the building in construction, employee was exposed to a fall of 7'-6". The employee used a personal fall arrest system with 4 foot lanyard with shock absorber. In case of a fall, the employee would hit the ground, on or about December 11, 2013.
