29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder (handheld angle grinder) were not provided with safety guard having a maximum exposure angle of 180 degrees and located between the operator and wheel:    (a) Fabrication Shop. On or about October 22, 2012, Metabo 4420 Quick 120V portable handheld angle grinder was used by employees during welding and fabrication operations. Employees were exposed to lacerations where the grinder was not equipped with an adequate safety guard.    There is no abatement certification or documentation required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by the unguarded portion of the saw blade:    Door assembly arena; north-east corner. On or about October 22, 2012, employees were exposed to cuts and lacerations from an unguarded portion of the blade on the Kalamazoo horizontal metal cutting band saw during steel beam cutting operation. Approximately 23 inches of the blade were unused and unguarded.    The Youngs Custom Ag Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 312728843, citation number 01, item number 003 and was affirmed as a final order on April 15, 2010 with respect to a workplace located at 32625 1360 North Ave., Spring Valley IL 61362.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition:    On or about October 22, 2012, employees working in the following areas were exposed to electrical shock hazards;  (a) Fabrication Shop; Metabo 4420 Quick 120V portable handheld angle grinder, the electrical power cord used by the employees during grain head parts fabrication has been damaged and wrapped with electrical tape.    (b) Assembly area; Milwaukee Magnum Heavy Duty 1/2" drill, the electrical power cord the electrical power cord used by the employees during grain head assembly operation has been damaged and wrapped with electrical tape.    The Youngs Custom Ag Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.242(a), which was contained in OSHA inspection number 312728843, citation number 01, item number 006a and was affirmed as a final order on April 15, 2010, with respect to a workplace located at 32625 1360 North Ave., Spring Valley, IL 61362.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

(b) Assembly area; Milwaukee Magnum Heavy Duty 1/2" drill, the electrical power cord used by the employees during grain head assembly operation has been damaged and wrapped around with a black electrical tape.    The Youngs Custom Ag Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.334(a)(2)(i) which was contained in OSHA inspection number 312728843, citation number 01, item number 006b and was affirmed as a final order on April 15, 2010, with respect to a workplace located at 32625 1360 North Ave., Spring Valley IL 61362.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
