On October 25, 2012, an employee was not provided cave-in protection while working in an unprotected trench, exposing the employee to the hazards associated with working in a trench measured at five (5) feet ten (10) inches in depth. H&R Construction, Inc. was previously cited for a similar violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in inspection number 312723638, citation number 1, item number 1, and was affirmed as a final order on March 30, 2009, with respect to a work place located at 848 Countyline, Hinsdale, Illinois. 29 CFR 1926.200(g)(1):     Construction areas were not posted with legible traffic signs at points of hazards:      On October 25, 2012 at a worksite in Hinsdale IL, H&R Construction did not conform to section 6G.03 of the Manual on Uniform Traffic Control Devices (MUTCD) by placing a ROAD CONSTRUCTION AHEAD signs on the east and west bound lanes of Hickory St., exposing employees to the hazards associated with working in the traveled way.    H&R Construction, Inc. was previously cited for a similar violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.200(g)(2) which was contained in inspection number 312723638, citation number 1, item number 2, and was affirmed as a final order on March 30, 2009, with respect to a work place located at 848 Countyline, Hinsdale, Illinois.
