On or about August 14, 2019, at the above addressed jobsite, employees working in a trench were not wearing hard hats to protect against falling materials and debris.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c) of this section. a. On or about August 14, 2019, at the above addressed jobsite, employees were conducting a new water service connection in a trench measured at six (6) feet two (2) inches in depth. The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.
