29 CFR 1926.501(b)(1):     Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.    At a jobsite in Sycamore, Illinois an employee was working on a flat commercial roof approximately 30 feet above grade installing a section of gutter without the use of fall protection.    Tip Top Roofing & Construction Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(1) which was contained in OSHA inspection number 311253447, citation number 1 , item number 2, and was affirmed as a final order on 11/19/08, with respect to a workplace located at Route 23, Sycamore, Illinois.    Tip Top Roofing & Construction Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 312730542, citation number 1 , item number 2 , and was affirmed as a final order on 01/29/10, with respect to a workplace located at 620 W. Roosevelt Road, Wheaton, Illinois.    Abatement documentation is required fo t this item in accordance with the requirementws of 29 CFR 1903.19(d)

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At a jobsite in Sycamore, Illinois the rails of an extension ladder did not extend a minimum of 3 feet above the landing surface.    Tip Top Roofing & Construction Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 311253447, citation number 2 , item number 6, and was affirmed as a final order on 11/19/08, with respect to a workplace located at Route 23 SYcamore, Illinois.    Tip Top Roofing & Construction Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1926.1053(b)(1) which was contained in OSHA inspection number 311250203, citation number 1, item number 5, and was affirmed as a final order on June 25, 2008, with respect to a workplace located at 1500 West Gate, Rochelle, Illinois.    There is no abatement cerrtification or documentation required for this item.
