29 CFR 1926.503(a)(1): The employer does not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: The employer does not provide training to recognize and minimize the hazards associated with working at heights to exposed employees. This most recently occurred on August 12, 2014, at the above addressed jobsite. Employees are not trained to recognize and minimize hazards associated with falls. To abate this hazard in the future, the employer must provide fall protection training to employees and ensure employees understand the training received. Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from falls and ensures adequate training of employees.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: On or about August 12, 2014, at the above addressed jobsite, employees were performing residential roofing operations where a competent person had not inspected the site for unsafe conditions, exposing employees to the hazards associated with residential roofing operations. Tip Top Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 912723, citation number 1, item number 2 and was affirmed as a final order on May 21, 2014, with respect to a workplace located at 435 S. Tenth St., DeKalb, IL 60115. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
