29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder    (a) Arteaga Roofing, LLC @ 22532 Indora Dr., Aurora, CO 80026:  On or before October 1st, 2012, the exposing employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  Employees were using a portable extension ladder for access to the roof. Employees were exposed to a fall of approximately 10' while attempting to use the ladder.
