29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: (a) Location, 7 Bedford Street Billerica, Ma, 01821 On or about 10/31/12, employees were not protected from falling greater than 10 feet while working on a ladder jack scaffold. Luis Quindi DBA Speedy Gonsales Roofing was previously cited for violation of this Occupational Safety and Health standard or its equivalent standard 1926.451(g)(1)(i), which was contained in OSHA Inspection Number 314703299, Citation Number 01, Item number 001B, issued on 08/27/2010, and was affirmed as a final order on 10/09/10 with respect to a workplace located at 2 Amandas Way Groton, MA, 01450.
