29 CFR 1903.19(c)(3): The employer did not submit certification that abatement is complete for each cited violation, in addition to the information required by paragraph (h) of this section, the date and method of abatement and a statement that affected employees and their representatives have been informed of the abatement.  On or about November 19, 2015, the employer failed to submit an abatement certification for the following citation issued on October 30, 2015;  1. Citation 1, Item 1: 29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Location: 4 Weetamo Way, Westford, MA  This violation was most recently observed on 7/15/2015 at a jobsite located at 4 Weetamo Way, Westford, MA 01886 when installing roofing shingles with pneumatic nail guns, the employees were exposed to eye impalement and laceration hazards where eye protection was not utilized. To abate this violation, the employer must ensure that safety glasses are utilized to protect employees from eye impalement and laceration hazards while utilizing pneumatic nail guns.

Citation 1, Item 2c: 29 CFR 1926.452(k)(1): Platforms exceeded a height of 20 feet (6.1 m)  Location: 4 Weetamo Way, Westford, MA  This violation was most recently observed on 7/15/2015 at a jobsite located at 4 Weetamo Way, Westford, MA 01886 where roofing shingles were being installed on an 8-pitch roof, employees were exposed to falls greater than 20-feet, 3-inches while working from a ladder jack scaffold.  To abate this violation, the employer must ensure that ladder jack scaffolds do not exceed the height of 20 feet.
