The worker was exposed to a fall hazard.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: JOB SITE: 2034-2036 Quiet Stream Drive, Maryland Heights, MO 63043, at this work site, the employer failed to provide a competent person to conduct frequent and regular inspection of the job site to control or eliminate job hazards associated with the work activity at this job site. The workers were installing new roof shingle on a 6:12 pitch roof, approximately 15 feet 1 inch eave to ground level without any form of fall protection (Alternative or Conventional), working around a skylight without any form of fall protection or cover and the ladder side rails were less than 3 feet above the upper landing surface. NATIONAL DESIGN & EXTERIORS, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard of 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 314281759, citation number 1, item 1, issued on 7/12/2010 and was affirmed as a final order date of 8/05/2010, with respect to a workplace located on 1621-1629 Marriott Lane, Barnhart, MO 63012
