(a) On or about October 31, 2012, at 9230 E 47th ST Kansas City Missouri 64133, machine shop employees were exposed to a hazard from welding metal together without using a protective required face piece.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) (a) On or about October 31, 2012, employees at 9230 E 47th ST Kansas City Missouri 64133 , it was discovered during inspection that the employer had a not provided training for employees who are required to operate a Hyster fork truck.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. (a) On or about October 31, 2012 at 9230 E 47TH ST Kansas City Missouri 64133 maintenance employees were exposed to a hazard by operating fork lifts in need of repair. No system was in place to identify euipqment in need of repair, fork lifts were being used inspite of apparent visual defects.

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. (a) On or about October 31, 2012 at 9230 E 47 TH St KAnsas City Missouri 64133 A hazard exists in the warehouse whereby operators were not required to conduct daily inspections on fork lift trucks that everyone operates. Operators could not determine if operating controls to include steering, braking, lift controls were functioning as intended.
