29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about November 7, 2012, during commercial framing activities, the employer did not ensure employees were protected from fall hazards approximately 9 feet at the eave, by ensuring fall protection system/device was utilized.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:    On or about November 7, 2012, during framing activities including but not limited to installing plywood sheathing, the employer did not ensure all employees on the roof were attached to an individual anchor point capable of supporting 5000 pounds. Employees were exposed to a fall hazard approximately 9 feet at the eave and 28 feet at the peak.
