29 CFR 1910.23(a)(4): Every skylight floor opening and hole was not guarded by a standard skylight screen or a fixed standard railing on all exposed sides:    VA Livermore, Building 90, CLC Roof: On November 20, 2012, and at times prior thereto, the fourteen skylights above resident porches were not adequately guarded to prevent employees from falling over five feet before being stopped by wooden beams.
