29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Weinig Hydromat 23C Moulder: On November 6, 2012 and at times prior thereto, employee(s) were using a Hydromat 23C to make cuts into lumber. Employees were tasked to make adjustments to the wheels while the machine was operating. Employees were exposed to amputation hazards due to unguarded rotating parts.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: Prime line feed conveyor: On November 6, 2012 and at times prior thereto, employee(s) were exposed to an unguarded rotating shaft from the conveyor belt drive shaft while feeding lumber into the prime line. The shaft projected out approximatley 2 inches and was approximately 2 inches in diameter.
