29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. On the 4th floor of the jobsite, a blue extension cord with a hole in the insulation had visible arcing through the hole and was not inspected prior to each days use, thereby exposing employees to a contact hazard. b. On the 4th floor of the jobsite, there were cracks to the female end of a blue extension cord with visible copper conductor through the cracks and it was not inspected prior to each days use, thereby exposing employees to a contact hazard. c. On the 4th floor of the jobsite, a blue extension cord was cut or abraded through the outer insulation and it was not inspected prior to each days use, thereby exposing employees to a contact hazard. d. On the 4th floor of the jobsite, a Makita circular saw had a plug with the strain relief clamp pulled away from the cord and was not inspected prior to each days use, thereby exposing employees to a contact hazard.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. On the 4th floor of the jobsite, a blue extension cord had a hole in the insulation that was visibly arcing through the hole, thereby exposing employees to a contact hazard. b. On the 4th floor of the jobsite, the female end of a blue extension cord was cracked with visible copper conductor through the cracks, thereby exposing employees to a contact hazard. c. On the 4th floor of the jobsite, a blue extension cord was cut or abraded through the outer insulation, thereby exposing employees to a contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On the 4th floor of the jobsite, the strain relief clamp on a Makita circular saw was pulled away from the cord and exposed the interior conductor insulation, thereby exposing employees to a contact hazard.
