(a) Mondelez Global LLC, 2221 N. Front Street, Toledo, Ohio: On or about November 26, 2012, the employer did not ensure that the written housekeeping program established the frequency and methods determined best to reduce accumulations of fugitive grain dust on exposed surfaces. Accumulation of grain flour was observed under the flour sifter.
