29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service: a) Polishing and Grinding Area: A flexible extension cord, plugged into a GFCI outlet and a hand grinder, had visible damage to the outer jacket and insulation, on or about 11/28/2012. No abatement certification or documentation required.
