29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part:    a.  On or about December 4, 2012, at the above addressed jobsite, employees performing masonry operations from a tubular welded frame scaffold, sixteen (16) feet above the ground, were exposed to injuries from falls due to the lack of guardrails.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a. On or about December 4, 2012, at the above addressed jobsite, employees performing masonry operations from the roof were exposed to falls, of fifteen (15) feet, created by the presence of unprotected sides.
