29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a power industrial truck: On July 7, 2014 employees were exposed to the hazards associated with working around a rough terrain fork truck that was operated by an employee who was not trained or certified. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On July 7, 2014, employees were not protected from a fall while working from a metal frame scaffold exposing employees to a fall of approximately 35 feet. Denali Masonry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 766946, citation number 01, item number 01 and was affirmed as a final order on January 5, 2013, with respect to a workplace located at 634 W. Hickory Hinsdale, Illinois. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
