DEPARTMENT OF VETERANS AFFAIRS, VISN 21 WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD (29 CFR 1910.303(b)(2) AS REQUIRED BY 29 CFR 1960.8(b)) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 109385 NOTICE NUMBER 1, ITEM NUMBER 6, ISSUED ON 05/07/2012 AND CONFIRMED AS A FINAL ORDER ON 06/12/2012 WITH RESPECT TO A WORKPLACE LOCATED AT 150 MUIR ROAD, MARTINEZ, CA 94553.
