29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about December 12, 2012, at the above addressed jobsite, an employee was installing a water line in a trench measured at six (6') feet in depth. The trench was not provided with cave in protection, thereby exposing the employee to cave in hazard. THE MOSS SEWER & WATER, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 108266, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 23, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 207 SOUTH STREET, ELMHURST, IL 60126. THE MOSS SEWER & WATER, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315419630, CITATION NUMBER 1, ITEM NUMBER 4 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 2, 2011 WITH RESPECT TO A WORKPLACE LOCATED AT 1250 CAMPGROUND ROAD, DES PLAINES, ILLINOIS, 60016. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
