29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): On and before 12/03/2012 while completing E.I.F.S./stucco work to the exterior of a Holiday Inn Express, workers accessing scaffolding that was not fully planked, were exposed to a potential fall hazard. The scaffolding measured approximately 27 feet, from the top of the scaffolding to ground level.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. Scaffolding four (4) bucks high and at least eight (8) bays wide, used by employees completing E.I.F.S./stucco work to the exterior of a Holiday Inn Express, was not properly restrained from tipping.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: Scaffolding relocated from the front of a Holiday Inn Express to the rear of the building and used by employees completing E.I.F.S./stucco work, was not inspected before the start of the work shift. On 12/03/2012 scaffolding four (4) bucks high and at least eight (8) bays wide was not properly restrained from tipping, and thus exposed workers to a potential fall hazard.
