29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about March 30, 2018, employees were exposed to a struck by hazard. The employees were installing stucco materials on a new apartment building located at 3600 Park East Drive, Beachwood, Ohio 44122. Employees were working on the ground under other employees and materials without wearing protective helmets.

The employees were installing stucco materials on a new apartment building located at 3600 Park East Drive, Beachwood, Ohio 44122. The employees were working on upper levels of the scaffold that were not fully planked.

The employees were installing stucco materials on a new apartment building located at 3600 Park East Drive, Beachwood, Ohio 44122. The employees were working on upper levels of the scaffold without a complete guard rail system in place.
