M & W Construction, Inc. dba Special Maintenance Roofing worksite located at 451 N. Cable Rd., Lima, Ohio: On or about December 11, 2012, the employer did not ensure the ladders supporting ladder jacks were secured to prevent the ladders from slipping. The ladders were located on the top of a plactic tarp and not secured.

Cable Rd., Lima, Ohio: On or about December 11, 2012, the employer did not ensure employees were protected from falls while performing weatherproofing activities on the low sloped commercial structure. Employees were exposed to a potential fall distance of approximately 9.00 feet.
