Buckeye Hone Company - On or about 12/14/12, employees performed maintenance and servicing work to equipment including, but not limited to, honing machines, centrifuge, and bandsaws. The lockout/tagout procedures for the machinery did not include instructions on how to drain stored energy, energy sources and magnitudes, and steps to release the lockout.
