Buckeye Hone Company, honing machine #12 area, on or about October 17th, 2018 and times prior thereto: Employees utilized a flexible (extension) cord to power their cellular telephone charger and cellular telephone in the honing #12 area of the facility. The flexible cord was wrapped around a metal beam exposing the cord to potential damage during use.
