Buckeye Hone Company - On or about 10/17/18, and at times prior thereto, the Lincoln arc welder did not have covers over the terminals. The caps were missing. The welder is used to perform repair work of machinery in the facility.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replaced: A.) Buckeye Hone Company - On or about 10/17/18, and at times prior thereto, the Lincoln arc welder exposed wires where the insulation had been pulled away from the terminal area. The welder is used to perform repair work of machinery in the facility. B.) Buckeye Hone Company - On or about 10/17/18, and at times prior thereto, the Lincoln arc welder had areas on the ground where the insulation had been cut creating exposed wires. The welder is used to perform repair work of machinery in the facility.
