29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    Maintenance Shop, Wash Rack, Soap Mixing Room:  On or about 19 December 2012 and times prior thereto, the 120 volt electrical junction box missing was a cover plate.  This condition exposed employees to an electrical shock hazard.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.    (a)  Chassis shop, middle row:  On or about 19 December 2012 and times prior thereto, the extension cord roller exhibited strain relief on the electrical plug exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.      (b)  Chassis shop, south bay area:  On or about 19 December 2012 and times prior thereto, the extension cord roller exhibited strain relief on the electrical plug exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.      (c) Maintenance shop, Reefer repair area:  On or about 19 December 2012 and times prior thereto, the reefer cord exhibited strain relief on the electrical plug exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.      (d) Maintenance shop, Wash rack bay:  On or about 19 December 2012 and times prior thereto, the blue light assembly exhibited strain relief on the electrical plug exposing wires.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.334(a)(2)(i):     Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).    a)  Chassis shop, middle row:  On or about 19 December 2012 and times prior thereto, a portable grinder was missing a ground pin.  This condition exposed employees to a 120 volt electrical shock hazard.      (b) Maintenance shop:  On or about 19 December 2012 and times prior thereto, the extension cord roller female end in had evidence of burn through.  This condition exposed employees to a 120 volt electrical shock hazard.      (c) Maintenance shop, hand tool storage:  On or about 19 December 2012 and times prior thereto, the Milwaukee saw was missing a ground pin.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1917.122(d):     The minimum width of any employee exit shall be 28 inches (71.12 cm).    Chassis shop, old boiler room:  On or about 19 December 2012 and times prior thereto, the exit door would only open to twenty inches.  This condition exposed employees to smoke inhalation and/or fire hazards from delays in exiting the building.

29 CFR 1917.152(e)(2)(ii):     Arc welding or cutting cable was used when it was not free from repair or splice for 10 feet (3 m) from the electrode holder.    Maintenance shop:  On or about 19 December 2012 and times prior thereto, the Thermal Dynamics Professional welder #18 had a stinger with exposed wires covered with common black electrical tape.  This condition exposed employees to an electrical shock and burn hazard.

29 CFR 1917.154:     Employees shall be protected by chip guarding and personal protective equipment complying with the provisions of Subpart E of this part during cleaning with compressed air. Compressed air used for cleaning shall not exceed a pressure of 30 psi. Compressed air shall not be used to clean employees.    Maintenance shop:  On or about 19 December 2012 and times prior thereto, employee was using an air nozzle without appropriate 30 PSI deflection tip installed.  Shop air was measure at approximately 120 PSI.  This condition exposed employees to air embolism/laceration hazards if they contacted their skin.

29 CFR 1917.152(c)(3):     Fire extinguishing equipment suitable for the location was not immediately available or maintained in readiness for use at all times during the performance of hot work.    Chassis and Maintenance shop area:  On or about 19 December 2012 and times prior thereto, the fire extinguishers in the chassis shop and two in the maintenance shop were overdue on their monthly inspections.  This exposed employees to burn hazards.
