29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling   (a) Pier 3, Maintenance shop, Tire shop:  On or about 12 August 2014 and times prior thereto a 120 volt, relocatable power tap was mounted to the side of the work desk.  This condition of use was contrary to the associated UL listing and labeling when using the equipment and exposed employees to a smoke and inhalation hazards in the event of RPT failure.   (b) Pier 3, Maintenance shop, Parts room:  On or about 12 August 2014 and times prior thereto a 120 volt, relocatable power tap was mounted to the shelf.  This condition of use was contrary to the associated UL listing and labeling when using the equipment and exposed employees to a smoke and inhalation hazards in the event of RPT failure.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.   Pier 3, Maintenance shop, store room:  On or about 12 August 2014 and times prior thereto a the 120 volt, electrical outlet missing a knock out.  This condition exposed employees to an electrical shock and burn hazard.
